Your Core Responsibilities:

• Conduct comprehensive in-home physical or occupational therapy assessments
• Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs)
• Provide hands-on therapeutic interventions and progress monitoring
• Educate clients, families, and caregivers on safe transfer techniques and home modifications
• Maintain clear, reliable communication with families and our clinical coordination team

✔️ What We Are Looking For:

• Active Massachusetts Physical Therapist (PT) or Occupational Therapist (OT) license (Required)
• A compassionate, professional, and dependable clinical work ethic
• Experience in home health, rehabilitative centers, or assisted living environments is preferred
• Strong communication, paperwork management, and teamwork skills
• Reliable transportation to attend scheduled client visits on time
