Job Title: Physical Therapist, Home Health/Hospice
Location: Grants Pass OR, 97527
Contract: 3 months (potential for extensions)
Shift: 8am - 4:30pm; 40 hours per week
Description:
What are the top responsibilities for this position:
Perform Physical Therapy visits including Start of Care (SOC), initial PT evaluations, recertifications, Resumption of Care (ROC), and required supervisory visits.
Develop, implement, and modify individualized treatment plans to improve patient mobility, strength, balance, and functional independence.
Accurately document visit notes, outcomes, and productivity metrics in the HomeCare Homebase (HCHB) EMR, meeting productivity expectations of 30 points per week.
Educate patients and caregivers on exercise programs, safety, fall prevention, and use of assistive devices in the home environment.
Coordinate care with physicians, nurses, and interdisciplinary team members to ensure continuity and quality of patient care.
